Try starting with a sphere and use the brush system in max to sculpt it into place, worry about the topology later on

I always use edge extrusion but that's just because I find it easier to control, normally I would start at the feet.

Seems to me that you are fighting with the technical side and not really "feeling the flow" using NURBS, I suggest you use polygon modeling for a start
